Dementia Care Programs
Overview
Commonpoint Queens has several programs and support services, led by licensed social workers, to accommodate community members at different stages of Memory Loss.
Adult Day Care Family Respite Program

Program takes place Monday through Friday from 10:00am to 2:00pm with customizable schedule options.
The Family Respite Program provides adult day care to older adults with Alzheimer’s disease or related memory disorders. The daily activities, designed to foster socialization, mental stimulation and to improve quality of life include:
- Educational programs
- Chair exercise
- Reminiscence and mental enhancement opportunities
- Music and crafts programming
- Current events discussions
- Intergenerational activities
Family members can take advantage of four weekly support groups which are available to help cope with the strain of care-giving. Clients are charged on a sliding scale basis. Limited transportation is available. Pre-admission screening is required. Kosher lunch is served.
Early Stage Dementia Care Program

Program takes place Tuesdays and Thursdays from 10:00am to 2:00pm
The Early Stage Dementia Care Program offers a twice weekly support program to assist people with newly diagnosed with Alzheimer’s disease and other memory disorders, and who are aware of their memory loss. Members participate in:
Family members can take advantage of four weekly support groups which are available to help cope with the strain of care-giving. Clients are charged on a sliding scale basis. Limited transportation is available. Pre-admission screening is required. Kosher lunch, snacks and hot drinks are provided.
*NEW* Younger Onset Program

The Younger Onset program is a weekly confidential support group for 60’s and younger with a diagnosis of early onset memory loss. The group will provide an opportunity to connect and build relationships with others facing the unique challenges of this diagnosis. The group meets Wednesdays from 1:00 to 3:30pm. An intake interview is required.
The Group is facilitated by a Licensed Social Worker providing:
- Supportive counseling
- Sharing resources & information
- Coping & stress management skills
- Activities designed to strengthen and enhance memory
- Strategies to build upon strengths and improve feelings of competency and well being
Transitions Program

The Transitions program is designed to address the needs of those individuals experiencing moderate memory loss. The group meets on Tuesdays from 10:00am to 2:00pm. A kosher lunch is served. An interview and evaluation is required.
The Transitions Program provides an opportunity to engage in:
- Thoughtful discussions
- Stimulating cognitive activities
- Socialization in a warm and supportive environment
Our goal is to offer a comfortable and friendly social environment, challenge the mind, increase self- confidence and improve quality of life by reinforcing strengths.
Caregiver Support Services
If you are struggling with caring for a partner or family member with a diagnosis of Dementia/Alzheimer’s… Remember YOU ARE NOT ALONE!
We offer bi-weekly caregiver support groups focused on:
- Strengthening communication, problem solving and coping skills
- Achieving balance and promoting self care
- Short term individual counseling as needed
- Information and referral services

*NEW Caregiver Support Group for Adult Child Caregivers
The Commonpoint Queens Memory Care Services team is launching a caregiver group designed specifically for adult child caregivers who are struggling with caring for a parent with Alzheimers Disease or dementia. The goal of this group is to provide a safe space to share with others going through a similar experience, juggling caregiving responsibilities while managing their own lives.
The Group focuses on:
- Strengthening communication, problem solving and coping skills.
- Achieving balance and promoting self care
- Short term individual counseling as needed
- Information and referral services
